 Job Title: RN PPS Coordinator/Case Manager


Job Summary: Performs all duties under the direct guidance of the PPS coordinator. Contacts insurance companies and obtain pre-certification / authorization for patient’s admission to the Acute Inpatient Rehab Department and will keep an open communication with the Acute Hospital discharge planners regarding insurance needs/issues. Assist Senior PPS/Case Manger in discharge planning in the rehab department; attends weekly IDT meetings to discuss patient status and probable needs. Performs daily coordination of management and functions as Case Management. Will work closely with the Discharge Setting Coordinator and Clinicians performing prescreens making sure transition from acute setting to rehab is smooth. Will provide coverage for the Discharge Setting Coordinator when they are out conducting internal and external chart assessments as needed. Will monitor all required documentation related to the Rehab Prescreen ensuring CMS requirements are met. Will complete the Inpatient Rehab Facility Patient Assessment Instrument(IRF-PAI), UDS information and transmitting information to CMS within the defined timeline as outlined in the Federal Register, will review medical records to ensure appropriate documentation occurs to support findings noted on the IRF-PAI. Will enter patient data into Rehab Data program. Will exemplify sound clinical judgment, experience, and expertise in area of rehabilitation. Also requires basic knowledge and operational skills of computer and office equipment.


Must have excellent oral and written communication skills, possess the ability and desire to work with individual of all races, ages, lifestyles, disabilities and cultures and willing to work as a member of the total healthcare team.


Reporting Relationship: Senior PPS Coordinator/Case Manager








Requirements: : 1) Graduate of an accredited School of Nursing. 2) Current Texas RN licensure 3) Minimum of 5 years experience in acute care setting in nursing and/or case management 4) CPR certification 5) ACLS 6) PALS 7) Basic arrhythmia/telemetry training

Must have current licensure/certifications upon hire.

New Hires will have 60 days to acquire the required Basic Life Support (CPR, ACLS, PALS, BLS, and/or any other life support certifications required for the job).

Universal precautions must be observed and protective devices worn by the employee when performing a task that is likely to cause exposure.
