HOME HEALTH REGISTERED NURSE

SALINE COUNTY JOB DESCRIPTION

REPORTS TO : Home Health Coordinator

 

POSITION SUMMARY  

Provides intermittent skilled nursing to patients in their place of residence in accordance with Agency policy and procedure and State and Federal regulations.

 

REGISTERED NURSE FUNCTIONS 

Performs thorough head-to-toe assessments on all patients when starting their Home Health care. Obtains vital signs with each visit and develops care plans that allow patients to remain in their home. Assists with medication reconciliation, pre-filling medication boxes, calls in refills of needed medications and communicate with Doctors regarding response to medications and reactions or side effects. Discusses missing medications, abuse of medications by patients or caregivers. Administers injections subcutaneous and intramuscular. Administers seasonal injections such as flu vaccines and pneumonia vaccines. Obtains laboratory specimens via venipuncture, PICC lines, Hickmans and Ports weekly, monthly and as needed. Delivers lab specimens to Doctors and follows up on results. Administers IV medications along with saline and heparin flushes. Monitors for drug reactions and side effects. Educates and teaches patient and family members how to do administration independently. Performs dressing changes to invasive lines in accordance with policies. Assists with insulin injections to include education on cleanliness, proper techniques to perform blood sugar testing along with educating patients on how to utilize glucometers, cleanliness need with finger sticks, appropriate blood sugar ranges and signs and symptoms of hypoglycemia and hyperglycemia. Provides toenail trimming for diabetic patients or those on blood thinners. Communicates with persons throughout the community such as Doctors, families, pharmacies, Agency on Aging, physical, occupational and speech therapy. Conveys assessment findings and changes in patient’s conditions to Doctor. Advocates for patients welfare. Assesses all patients admitted to Home Health for depression and follows up with necessary interventions. Provides support and resources to encourage improved mental health. Promotes safety. Provides wound care daily, every other day, weekly or as ordered by Physician. Assesses wounds for color, odor, drainage, measurements and most beneficial dressing needed. Includes wound VAC dressing changes which require certification due to complexity of the wounds. Changes indwelling foley and suprapubic catheters on a regular schedule and as needed. Irrigates catheters as needed. Sterile techniques maintained. Educates patients and family members to care for catheters and supplies. Monitors for signs and symptoms in decline with physical and mental health. Reports any signs or symptoms of infection to patient’s Doctor. Triage, on-call nights, weekends and holidays. Acts as a resource Case Manager by assisting patients set up medical and mental health care. Provides education to patients and families regarding pressure ulcer prevention, fall preventions, disease prevention, emergency preparedness and safety in and out of the home. Schedules and coordinates physical, occupational and speech therapy evaluations and re-evaluations per Doctor’s order and to be in compliance with Medicare regulations and changes by providing forms and monitoring the number of therapy visits required for patient’s plan of care. Determines homebound status and patient compliance as required by Medicare. Electronically documents all visits, phone calls, Doctors orders (every 60 days) and necessary information. Completes Doctors orders in a timely manner. Determines payer sources and services provided by insurance providers. Develops care plans to establish goals and necessary nursing interventions. Identifies discharge planning needs and implements prior to discharge. Communicates with Doctors and obtains orders as required for all skilled care. Administers medications and treatments as prescribed by Doctor. Completes all paperwork necessary for Agency policies and Federal and State laws and regulations. Teaches Home Health Aides appropriate wound care and how to apply compression stockings. Supervises Home Health Aides every 2 weeks to 60 days. Supervises Aide skills by direct supervision as needed for new procedures.

 

ADDITIONAL RESPONSIBILITIES

Has knowledge of and compliance with Department policies and procedures. Must complete the National Incident Management System training. Maintains certification in CPR and yearly blood-borne pathogen training. Reports for work during public health emergencies as needed. Serves on Boards and committees as needed to promote public health as well as the Home Health Program. Job requires weekend and/or evening work and be on-call. Perform other duties as assigned.

MINIMUM QUALIFICATIONS

  1. Must have a Bachelor’s Degree, Diploma or Associates Degree in nursing.
  2. Must meet licensing requirements by the Kansas State Board of Nursing for Registered Nurses and hold a current licensure for the State of Kansas.         
  3. Must have two (2) years experience in nursing preferably the Home Health field. 
  4. Must have good communication skills to work with Staff and public in a congenial manner.
  5. An ability to maintain a professional demeanor. 
  6. Must have basic knowledge of Excel, Word and other PC software. 
  7. A proven ability to plan and organize work. 
  8. Must be flexible and be able to adapt to different tasks.
  9. Must have a valid Driver’s License and must provide own transportation.

Ability to make sound judgment in making clinical decisions related to care of patients. Ability to read and understand medical information related to patient care. Ability to maintain a professional demeanor in challenging situations and to adapt appropriately to deal with individuals of varied socioeconomic backgrounds. May be subjected to highly volatile and stressful situations involving patients and/or families. 

 

ESSENTIAL PHYSICAL FUNCTIONS 

Determined Work Level: Level 3 – Medium Work  Exerting 20 to 50 pounds of force occasionally, and/or up to 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly. Ability to communicate by means of the spoken word. Ability to hear. Must be able to lift up to 50 pounds. Must have fine and gross motor ability. Must be able to operate computer, telephone and automobile.  Possible exposure to infectious pathogens, blood, body fluids and hazardous waste. May be exposed to a variety of work environments including outside of Health Department buildings in all weather conditions.
