RN Support Coordinator

Full-time Looking for a RN Support Coordinator in the Mt. Pleasant area to conduct assessments, reassessments, and support coordination for A+D MI Choice Waiver. Provides and coordinates case management services including the necessary documentation regarding client care according to the client plan of treatment. Successfully communicates with the office staff, support coordinators physicians and other agencies providing services to a client.

Requirements: RN License in the State of Michigan preferred. Positive, interpersonal skills. A thorough understanding of confidentiality and HIPAA principles.

Preferred Qualifications: Experience with DHS (Medicaid), one year of experience as a Registered Nurse is preferred Home health care experience, and demonstrated knowledge of health services and external regulations, and documented experience in quality management.
